AND AWNINGS THAT PROJECT OVER THE PUBLIC WAY MORE THAN SIX
INCHES
Complete the Building (Sign) Permit Application on line at the Inspectional Service Department or
il a personal computer. Inspectional Services Department is located at 831 Massachusetts Avenue

2. Projected Sign Application and Abutter Forms are available at City Clerk's Office, Room 103 City
Hall, 795 Massachusetts Avenue, [phone removed].
3. Complete application. Application must be signed by business owner and property owner black ink
only. You must measure and state the distance by which the sign or structure will project over the public
way. This application is signed under the pains and penalties of perjury. If you have difficulty
ascertaining the distance, you may wish to use a surveyor. Complete the abutter forms.
After completing steps (1) - (3), file the application with the City Clerk.
The completed application can be dropped off at the City Clerk's Office or e-mailed to
Iperez@cambridgema.gov.
4 The Clerk will place the application on the agenda for a City Council meeting for its consideration.
• During the months of September through June, the City Council meets every Monday at 5:30 p.m.
except for Monday holidays. During July and August, the City Council holds one summer meeting.
Applications must be received at the City Clerk's office on the Thursday prior to the Monday meeting.
• After the City Council has approved the petition, the City Cierk will send the petitioner an unexecuted
bond form.
5.
Petitioner must have the bond form executed by a Surety Company and then return it to this office.
6. When the executed bond is returned to the City Clerk's Office, the City Clerk will approve the application
that bond is acceptable. ISD will issue a building permit to the petitioner, so long as all building permit
requirements have been met. The petitioner can obtain the sidewalk obstruction permit from DPW.

The proposal for the Signage Proposal for Sweetgreen within 145 Broadway falls under Section 5.4 - CRA Board
Review of Preliminary Signage Designs of the MD District Guidelines and Review & Approval Process.
At the 8.5.2020 Design Review Committee (the "Committee") meeting representatives from Sweetgreen, O'Neil
Langan Architects, Batten Sign, and the design firm Ruggles Sign presented the Committee with color
schematics, illumination specifications, and size of the proposed signage. During this meeting the Committee did
not request changes or modifications to the proposed signage. At the 8.19.2020 CRA Board meeting, the signage
proposal for Sweetgreen (Attachment: A) was approved with the following motion by the CRA Board:
Approving the Sweetgreen retail signage proposal located at 145 Broadway within Parcel Two of the
Kendall Square Urban Renewal Area
The CRA approval is based on the following condition:
• The illuminated signage approved by the CRA Board will have a dimability feature, and upon installation
the CRA reserves the to ask for adjustments in brightness.
• All additional vinyl window treatments will need to be reviewed and approved by the CRA

1. Complete the Building (Sign) Permit Application on line at the Inspectional Service Department or
on a personal computer. Inspectional Services Department is located at 831 Massachusetts Avenue,

2. Projected Sign Application and Abutter Forms are available at City Clerk's Office, Room 103 City
Hall, 795 Massachusetts Avenue, [phone removed].
3. Complete application. Application must be signed by business owner and property owner black ink
only. You must measure and state the distance by which the sign or structure will project over the public
way. This application is signed under the pains and penalties of perjury. If you have difficulty
ascertaining the distance, you may wish to use a surveyor. Complete the abutter forms.
After compieting steps (1) - (3), file the application with the City Clerk.
The completed application can be dropped off at the City Clerk's Office or e-mailed to
lperez@cambridgema.gov.
4 The Clerk will place the application on the agenda for a City Council meeting for its consideration.
• During the months of September through June, the City Council meets every Monday at 5:30 p.m.
except for Monday holidays. During July and August, the City Council holds one summer meeting.
Applications must be received at the City Clerk's office on the Thursday prior to the Monday meeting.
• After the City Council has approved the petition, the City Clerk will send the petitioner an unexecuted
bond form.
5. Petitioner must have the bond form executed by a Surety Company and then return it to this office.
6. When the executed bond is returned to the City Clerk's Office, the City Clerk will approve the application
that bond is acceptable. ISD will issue a building permit to the petitioner, so long as all building permit
requirements have been met. The petitioner can obtain the sidewalk obstruction permit from DPW.

CONDITIONS OF CONTRACT
1. The above prices do not include state or local taxes which may be imposed by any govemmental body, permit fees or costs of time and expenses involved in obtaining
same, whether or not said permit applications are ultimately approved or not. In addition, Batten Bros., Inc. hereinafter referred to as "The Company", assumes no
responsibility for the subsequent maintenance of permits once obtalned.
2. Interest at 1-1/2% per month (18% per annum) will be charged on all overdue accounts.
3. Ground Clause - The supporting structure shall be embedded in a concrete base.
The concrete shall be installed on the basis of normal soil-bearing conditions, with
normal excavating conditions. If unusual soil-bearing conditions do exist, and the services of special equipment or other contractors are necessary, then the Customer
shall pay the costs of said contractor.
4. In the event that a sign is to be installed on or connected to a bonded roof and if the services of a roofing contracior are requined to install sign fastenings, then the
Customer shall pay the costs of said contractor.
5. Unless specifically stated otherwise, all necessary wiring and adequate branch circuits shall be provided to the Company by the Customer to a point within six (6) feet of
each sign location on the exterior of a building or as the Company may designate.
6. The contract price is based on the existence of normal masonry conditions. For purposes of this section, normal masonry conditions shall be pure masonry without foreign
objects either within or behind the wall. In the event normal masonry conditions are found not to exist, the Customer shall be responsible for any and all extra costs and/o
expenses incurred by the Company in drilling through said wall. On the walls other than masonry, the contract price is based on the assumption that no foreign objects
exist in the interior or back of the wall. In the event foreign obstacles are found to exist on the interior or back of non-masonry walls, the Customer shall be responsible for
any and all extra costs and/or expenses incurred by the Company in drilling through said wall.
7. The parties agree that it is the express obligation of the Customer to insure that the area on the rear of any wall where a sign is to be erected is sufficient to allow for the
installation of electrical or structural components. The Customer warrants that, in accordance with the requirements of law, accessibility to electrical components will be
maintained after installation of the sign.
8. Certificates of Workmen's Compensation and Public Liability will be fumished upon written request.
9. WARRANTY. The Company warrants that each sign of the kind and quality described in the applicable specifications and free from defects in material or workmanship
and guarantees each new sign for ninety days, and one year on specific parts, against all mechanical defects. Plastic faces, broken tubes, normal loss of brilliancy of
neon tubing andfor fluorescent lamps during periods of low temperature, bumed out lamps, electrical wiring and blown fuses are not included in warranty. Customer
agrees to pay for all maintenance or service calls which are not covered by warranty. The foregoing shall not be construed to include a warranty or guaranty on the part of
the Company against variations in color of any materials used or fabricated in any sign or any damaged due to an Act of God, or nature from wind or hurricane, vandalism,
accidant or other similar causes and not other warranty or guaranty express or implied, excepting title, shall exist. Subject to the foregoing, any complaint concerning the
quality, quantly or condition of any sign must be made in writing wilhin 30 days from the date of delivery and the failure to give such written notice shall thereafter be a bar
to any claim by the Customer. If a complaint is duly made as aforesaid, the Company shall have a reasonable opportunity thereafter to adjust the complaint.
10. DELIVERY. All delivery dates are approximate. The Company shall not be liable for delay due to causes beyond its reasonable control, such as Acts of God, vandalism,
accident, acts or circumstances of the Customer, acts of civil or military authorities, fires, strikes, floods, riots, delays in transportation, and inability due to causes beyond
its reasonable control to blain necessary labor, materials or plant facilities. In the event of any such delay, the date of delivery shall be deferred for a period equal to the
time lost by reason of delay.
11. INDEMNIFICATION. Customer authorizes the Company, its agents, servants and employees to erect or attach a sign or signs and electrical equipment and necessary
structures on the building or property specified in this Contract and the Customer agrees to indemnify and save harmless the Company, its agents, servants, or employees
from any charge, claim or action for damages arising out of said attachment of erection whether or not the Company, its agents, servants and employees have acted in a
negligent or unworkmanlike manner.
12. Prices quoted are based on current material and labor costs, but are guaranteed for 30 days from the date of quotation. In the event the Customer fails to accept this
contract within 30 days, the Company reserved the right to withdraw this offer of contract at any time thereafter. It is further agreed that in the event the Customer cancels
the contract after acceptance, the Company shall be enttled to damages, including the fair value of any work done and/or lost profits and any other damages resulting
from said cancellation. The Company shall have the right to appropriate any deposits given by the customer in partial or full satisfaction of said damage claim.
13. ASSIGNMENTS. Any assignments of this contract by the Customer without written consent of the Company shall be void.
